Our world is filled with forms. Unfortunatilly scouting is no different. We have tried to place all the forms that you might need in one location for you. Most of these forms have been designed to filled out right on your device and sent directly to Troop 978.

Health Forms A & B
BSA Offical Health Forms A & B. Adults and youth need to complete forms A and B for any overnight camping trips (all information on these forms is self-reported). These forms must be current (last 12 months).

Health Forms A, B, & C
BSA official Health Forms A, B, & C. Adults or youth participating in outings lasting 72 hours or more (including all BSA summer camp programs) will ALSO need to complete Medical Form Part C which requires a doctor’s signature. These forms should be accompanied by a current medical insurance card. Forms completed in July of the prior year will be applicable to the end of July (July 31st) of the current year.

All adult volunteers need to complete an Adult Application to be registered with the council and the BSA.

YOU MUST BE A REGISTERED ADULT TO ATTEND OWASIPPE. All adult volunteers need to complete an Adult Application to be registered with the council and the BSA.

EVERY adult attending camp must be current in their BSA Youth Protection Training.

This is the IL Child Support Form. PLEASE NOTE IT CAN TAKE UP TO 6 WEEKS FOR THE STATE TO COMPLETE THIS!!!!

This must be filled out by EVERY adult attending camp.
